Daily activity report form for childcare?

I just started an in-home daycare. I have been writing down info for the parents such as: how many diapers changed, wet or dirty, meals eaten, incidents....stuff like that. Does anyone know where I can get a good form for free so I dont gotta write it all down! I could make one on my own but I need ideas of format and stuff.   1. Try microsfot.com templates, they have lots of free downloadable templates. They might have one, or have a format that you can tweak to fit your need.
  2. use an excel spread sheet, you would be able to make differnt catagories and then write what you want to put in there. or search for templates that u can download.
  3. Take it from a mother. You don't have to be fancy, we understand that you have a busy life. Just make the form out with the different things that you need on it. Then throw in some cute little black and white pictures so it looks a bit daycareish Childs Name Date Teacher's name Class Naptime______________________________ Mealtime: I ate Some All None of my Breakfast Some All None of my Lunch Some All None of my AM PM Snack Diapertime I had _________BM's and _________ wet diapers today. I felt Happy Sad Inquizative Angry frustrated Sick I did or did not participate in activities._______ Something nice I did today._________________ You circle only what applys to the child and of course you can make it a little nicer looking if you want.
